Download, print, and then fill out all three forms. Remember to bring them to our office for your patient visit.
If you belong to a managed care plan that we participate in, you must show us your insurance card at the time of each visit. You then will only be responsible for your co-payment and any deductible.
If you belong to an HMO be sure to bring your referral with you at the time of your visit or ask your primary care doctor to fax it to us.
Call our office at least 24 hours before your appointment date to be sure we have received it. (We will be unable to see you without a referral.)
